7. Has your loved one experienced social or relationship problems due to their drinking or drug use and kept using anyway?

8. Has your loved one kept drinking or using drugs knowing that it has caused or worsened physical or mental health issues?

9. Has your loved one ever driven or operated machinery while under the influence of drugs or alcohol?

10. When your loved one attempts to cut back on or stop their drinking or drug use, do they experience uncomfortable physical or mental health symptoms (withdrawal)?

11. Has your loved one experienced diminished effects when they drink or use drugs compared to the past and/or have they needed more (drugs or alcohol) in order to feel the effects they’re seeking (increased tolerance)?
